The request of inclusion of a medicine will be carried out using the standard form, which is available at the Services of Pharmacy and on the intranet, and will be addressed to the secretary of the CFT.

Any doctor of the ICO centres will be able to make the request. The signature of the Head of the corresponding Service will also be requested. In the request will have to be indicated:
Indication for which it is requested / indication approved in Spain/EMEA/FDA
Dosage, posology and duration of the treatment
Estimation of the number of patients candidate at all the ICO centres
Suggested protocol or clinical guide that would be included
Clinical trials with this medicine at the centre (if they happen)
Summary of the evidence
